Organizations must establish an environment that allows mistakes. Too often people or employees are afraid of making mistakes because they will be quickly and swiftly dealt with. These same people are also afraid to report mistakes they see on a daily basis. Eighty percent (80%) of critical or serious events are due to human performance. Of those, 30% are due to individual weaknesses or errors, and the rest are due to organizational system deficiencies. Therefore, it is even more important to create and establish working environments that foster allowances for mistakes. The realization is – even the best people with in any organization will make mistakes.
